FHP provides shared infrastructure and administrative support to independent health practitioners. Each independent practitioner operating within the shared CharmHealth environment maintains their own HIPAA compliance policies, Notice of Privacy Practices, and patient record responsibilities. This Privacy Policy applies to FHP's organization and website operations.
2. Information We Collect
We collect information you voluntarily provide to us when exploring or using FHP services:
- Inquiry & Application Data: Name, professional degree/credentials, practice name, business email address, phone number, practice state, and answers to intake/fit questions.
- Directory & Profile Data: Practice description, website URL, headshot photo, specialty focus, and professional contact details provided for inclusion in the FHP Practitioner Directory.
- Membership & Billing Data: Payment processing details handled securely via integrated payment processors (e.g., Bluefin / Stripe / CharmHealth merchant services). FHP does not store full credit card numbers directly on our servers.
- Website Usage Data: Anonymized or aggregated analytics, IP addresses, browser type, referring pages, and device information collected via standard web server logs and cookies.
3. How We Use Your Information
We use the collected information for specific, operational purposes:
- Responding to membership inquiries and scheduling initial consultation calls.
- Provisioning access to the shared CharmHealth EHR facility, member Google Workspace environment, and member communications.
- Managing membership invoicing, service fee tracking, and account administrative support.
- Publishing public practitioner profiles in the FHP Directory (only with practitioner authorization).

4. Disclosure & Shared Infrastructure
We do not sell, rent, or trade your personal or professional data to third parties. We disclose information only as required to operate FHP services:
- Third-Party Service Vendors: Established vendors powering FHP infrastructure (such as CharmHealth, Google Workspace, Fullscript, and scheduling systems) process data strictly as necessary to deliver platform features.
- Legal & Regulatory Compliance: We may disclose information if required by law, court order, subpoena, or to protect the safety, rights, or property of FHP, its members, or the public.
5. Patient Data & Healthcare Information
Protected Health Information (PHI) created or received during patient care is stored within each independent practice facility in the CharmHealth EHR system under appropriate Business Associate Agreements (BAAs) and technical safeguards. FHP administrative and care management staff access facility data solely to perform authorized administrative, lab tracking, or operational tasks requested by the treating practitioner.
6. Security & Data Retention
We implement reasonable administrative, technical, and physical security measures to protect your information against unauthorized access, loss, or alteration. We retain practitioner information for as long as your membership remains active, or as necessary to fulfill legal obligations and maintain accurate business records.

You may request updates, corrections, or removal of your public FHP Directory profile at any time by contacting us. Members can update account and notification preferences directly through FHP administrative support or Google Workspace settings.
